12. Project/Activity summary (Titles: objectives, activities, achievements):
The Republic of Karelia is located in the north-western part of Russia. The total territory is 172,224 sq. km with the population of 800,000. The Republic has a workforce of 464,000 people of whom 45% are employed in the timber and pulp industry. About 20% of the working population is employed in machine building and the metal industry.

As a part of the neighbour country co-operation, a training programme was started in 1992 by Kuopio and Lappeenranta Regional Institutes of Occupational Health and Labour Protection Districts of Kuopio and Häme. The aim of the project was to evaluate and develop the occupational health and safety in the Republic of Karelia and build up a model of occupational health services in one factory. The Karelian partners in the project were the Ministry of Health, the Ministry of Labour the Sanitary-Epidemiologic Institute and AO Kondopoga pul and paper mill.

In the first stage some 30 occupational physicians and labour portection inspectors were trained in Petrozavodsk. The second stage was the advanced training course in Finland. The occupational health physician and labour protection inspector of AO Kondopoga pulp and paper mill was trained in Kuopio and Lappeenranta. Three occupational physicians were also trained at the same time. The course programme consisted of both lessons and group work concerning occupational health care, labour protection and work hygiene in Finland. The purpose of the training course was to educate the participants to assess risks at the work place and to measure the exposure to different physical and chemical factors with commonly used instruments.

For developing the working conditions at Kondopoga pulp and paper mill, the situation was examined by walk-through visits at different departments of the factory. After the visits it was decided to measure the physical factors at paper machines 1 and 8 (old and new machine). The measurements were done by Kuopio and Lappeenranta Regional Institutes of Occupational Health in co-operation with local personnel. The measurements were made using instruments of Institute of Occupational Health and local instruments at the same time. It was possible to compare the results of the different methods. The results were at the same level.

The labour protection districts of Häme and Kuopio made a precheck of construction plans for paper machine 10 and gave the examination report for the use of Kondopoga company. The examination was done according to Finnish regulations.

The main risks at the workplace were noise, risk of accidents and inadequate lighting. The noise level was 72-87 dB(A) in the monitoring rooms and 82-103 dB(A) in the machine hall. The use of ear muffs for workers in the machine hall was recommended and a construction plan for new monitoring rooms was given to the company. The audiogramms were recommended for inclusion in the periodical health examinations.

In general the exposure limits (TLV-values) were stricker in Karelia than in Finland. The incidense of work accidents and occupational diseases was 10 times lower in Karelia than in Finland. Despite these good indicators, the workers in the Republic of Karelia generally have higher exposure levels because of worse protection. At this level, legislation and regulation should be developed towards the European direction in the registration of occupational diseases and work accidents. Obviously the statistics are now underestimates of actual numbers of cases.
